New patients are always welcome.

Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. To schedule an appointment, please contact our office at (516) 256-2424.

If you have never been to our office before we will ask you to fill out a general patient history and registration form. In the interests of making your visit more convenient, the form is available online. Simply click here to download the form. Print it up, fill it out, and bring it with you to your appointment to save yourself time in the waiting room. Please note that you will need Adobe Acrobat® to view the patient form.

Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.

Our office provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week by calling

Billing and Payment Policies

Several insurance plans are accepted towards payment. We value our patient's insurance and extend our office courtesy to assist patients with their insurance towards payment for dental treatment.

We offer Citibank financial plan for dental services.

Financing
